New patients

Save time on your first date!

Print and complete the forms for new patients before your first appointment and deliver them to reception on the day of your appointment or you can send them to the email: Estudiosemory@gmail.com

If you do not have your completed forms, please arrive at the Emory Women Clinic at least 15 minutes before your scheduled appointment to complete the documentation. If a new patient's documentation is not completed in a timely manner upon arrival, we may need to accommodate other patients who arrive on time.
